Q: What are your office hours?
A: Monday through Friday: 8:00 AM to 5:30 PM
Closed Weekends and major holidays including Good Friday and the Friday after Thanksgiving


Q: What do I need to bring to my first visit?
A: We ask that you bring all of your current medications in their original bottles, we will need the name, address and telephone number of the pharmacy you want us to use to e-prescibe your medication. Bring all medical records related to your visit from your referring physician and current insurance card/s with you.


Q: Will I be reminded of my appointment?
A: You will receive a reminder call from our front desk staff the day prior to your scheduled appointment. If you do not receive a call, please contact administration using the "contact us" form on this website.


Q: Which forms do I need to fill out and bring with me to my first appointment?
A: You should complete the following forms before your appointment and bring them with you. Please call the office if you need assistance or have any questions: (1) Patient Information Sheet (2) PHI Release (3) Financial Policy.


Q: Where do I need to sign the "Financial Policy" form?
A: If you have Medicare or a Medicare Plan, you must sign all three sections. If you do not have Medicare or a Medicare Plan, you must sign the first and second section only.


Q: What do I do with the "Notice of Privacy Practices"?
A: This form explains your privacy rights. Please read it carefully and let us know if you have any questions. You do not need to bring this form with you, it is for your information only.


Q: What is the "PHI Release" form?
A: PHI Stands for protected health information. Your medical records are absolutely confidential. You, the patient, are the only one who can authorize any disclosure of your records. You have the right to restrict other people from accessing information about your medical care. The PHI Release is where you identify the person/s you give permission to receive information regarding your medical condition. We will not be able to speak to any person that you do not list on this form. This form can be updated as often as you wish.


Q: What do I need to pay on my first visit?
A: Most Insurance plans have a co-pay and you are responsible for that. If you are not insured, you must speak to the office before your appointment to find out what your cost will be. Payment is expected at the time of service.


Q: I changed my address/phone#/insurance. How do I notify the office?
A: You can call the office or fill out the "Patient Information Update" sheet and mail to us or bring it with you to your next appointment.


Q: How do I get my medication refilled?
A: Please call your pharmacy at least 3 days in advance of needing your medications refilled. The pharmacy will contact our office for the necessary information for your refill.
